Cerritos College provides alternative format for students who are eligible for this service. In order to maintain the integrity of the service offered and comply with the law, the following rules apply to students who use Cerritos College Alternative Media and students who use this system:

1) The student must currently be enrolled at Cerritos College in classes for the current semester or preparing for a licensing exam connected with their recent enrollment at Cerritos College.
2) The student must have a Verification of a Disability by an authorized professional and be registered with SAS.
3) Material provided to Cerritos College for the purpose of conversion to alternative format may sometimes be disassembled and returned to the student in its altered condition. Cerritos College will not be held responsible for any damage to the materials that occur as a result of conversion to an alternative format.
4) Textbook and required course material requests: The student must own, or have rented, a copy of the text or required book they are requesting in alternative format. Textbooks may not be borrowed, including those from libraries or instructors. Proof of purchase in the form of a receipt or rental contract may be requested at any time.
5) Resource/reference and other Cerritos College library material requests: If resource material from the Cerritos College Library is needed to complete a course assignment the student may either check the book out from the library, OR they must obtain the following information: title, author, edition number and/or name, and 13 digit ISBN number. The student must bring the book or required information to SAS to make the request for alternate format. The alternate format reference material will be loaned for the time needed to complete the assignment, and must be returned to SAS as soon as possible upon completion of the assignment. All resource material must be returned no later than the last day of the semester in which it was borrowed.
6) The student will not copy or reproduce any material provided by Cerritos College, nor allow anyone else to do so. Students will be provided with one alternative format copy for each material required for academic use. This material is copyrighted and is for the exclusive use of the student requesting the alternative format. Any further reproduction, distribution, or use is an infringement punishable by law and will result in disciplinary action by the Coordinator for Judicial Affairs.
7) The student agrees to return or destroy any Alternative Media provided by Cerritos College once the student no longer has legal possession of the original text or other material.
8) To submit a complaint regarding alternate text/media, please fill out the complaint form on SAS webpage. I have read, or verbally heard, this contract and understand the procedures and conditions of service listed above. My digital signature is my commitment to adhere to these responsibilities and terms.
